PATIENT ACCEPTANCE-TO-SERVICE

All Broward Home Health Services (agency) has a patient acceptance-to-service policy and information on how we accept referrals and any current limitations to patient acceptance.

Home Health Services offered include:

Skilled Nursing - specialties include, but are not limited to: Medical - Catheter, Diabetes, Hypertension (Cardiac), Lacerations/Pressure Ulcers/Wounds, Anemia, Pneumonia, Upper Respiratory Infection (COPD), Urinary Tract Infection, Venous Insufficiency. Psychiatric - Anxiety, Depression, Paranoid Schizophrenia, Psychosis, Schizoaffective.

Physical, Occupational and Speech Therapy - specialties include, but are not limited to: Arthritis, Cerebral Infarction, Cerebral Palsy, Dysphagia, Dementia, Bone Fractures, Lumbago, Multiple Sclerosis, Muscle Weakness, Muscle Wasting, Neuropathy, Osteoarthritis, Parkinson's Disease.

Medical Social Services provides social and emotional support/counseling to individuals and families in their homes, helping them navigate the healthcare system and access resources.

Home Health Aide provides personal care and activities of daily living.

All agency services are provided on a per visit basis.

When a patient is referred, acceptance-to-service will be based on the following:

Eligibility criteria such as insurance coverage, physician/non-physician practitioner orders, face to face documentation, homebound status, skilled need/medical necessity, etc.

Caseload, case mix, and staffing levels are appropriate to provide the needed care and services. Capacity of the agency to provide appropriate care/services and the ability to meet the anticipated needs of the patient.

Clinical staff have the appropriate skill set/competencies to provide needed and anticipated care.

If these criteria are not met, the patient referral will not be accepted and/or they will not be admitted for services. If it is determined that a patient is not appropriate for home care upon admission or throughout the course of care, the patient will be referred to alternative care options.

Potential limitations related to types of specialty services, service frequency or duration:

The agency currently does not accept the following types of specialty services or care: Infant/Pediatric (under 18), Maternity, Palliative, Pleural Drainage, Tracheostomy or Ventilator.

The agency does not accept patients that require more than daily visit frequency (2 or 3 visits daily) for each discipline. The agency will review such referrals to determine if more than daily visits is temporary and if the agency has the resources to accept such patients. The duration of services will be limited to physician/non-physician practitioner orders, insurance coverage and continued availability of agency staff.

The agency does not accept patients based on hourly services.
